To obtain The Sword in the Skull achievement, and the awesome transmogrification weapon that comes along with it, you first need to farm a bit of trash NPCs inside Forge of Souls or Pit of Saron 5man dungeons, on heroic or normal, in Icecrown, so as to obtain the Battered Hilt (Horde ver.)... or you could just browse the Auction House, maybe you'll get lucky and exempt yourself from some hours of farming.

[ALLIANCE ]:

[1] The Battered Hilt - visit Caladis Brightspear, in Icecrown, near Sindragosa's Fall[2] What The Dragons Know - go to Dragonblight, at the Wyrmrest Temple, at the very top of the tower, and ask Krasus "what he knows about ancient swords"; learn about the hilt's origins from Krasus and Kalecgos for quest completion and then go to Dalaran's Violet Citadel to speak to Arcanist Tybalin (he is located inside, near the stairs)[3] The Sunreaver Plan - recover the Sunreaver Orders from a Sunreaver Agent, located under the city, in the Underbelly[4] A Suitable Disguise - loot the Barrowed Tabard from the balcony of "The Threads of Fate" shop, in Dalaran[5] A Meeting With The Magister - speak to Magister Hathorel, inside Violet Citadel, to obtain the Ancient Dragonforged Blades[6] Return To Caladis Brightspear - go back to Caladis Brightspear, in Icecrown, and wait for him to examine the book

[1] Reforging The Sword - enter Pit of Saron, normal mode difficulty, search for 5 Infused Saronite Bars, then defeat Forgemaster Garfrost to obtain the The Forgemaster's Hammer and use it near Runeforge there

]Note]: If you can't enter Pit of Saron, then you haven't done the attunement quest chain, meaning the following one:

[a.] Inside the Frozen Citadel] - the first quest, offered by Apprentice Nelphi, which is walking around the inn / bank area in Dalaran, of the Silver Enclave[b.] Echoes of Tortured Souls - defeat the bosses of Forge of Souls, normal mode, and turn in the quest at Jaina that spawns inside the corridor, just before Devourer of Souls[c.] The Pit of Saron - enter Pit of Saron, normal mode, and complete it at Jaina, near the entrance (you can do the 1st quest of this phase now, but also continue this quest chain as well, for Halls of Reflection access)[d.] The Path to the Citadel - defeat Forgemaster Garfrost and free 15 Alliance Slaves[e.] Deliverance from the Pit - taken from Martin Victus, will ask you to defeat Scourgelord Tyrannus, the final boss inside the Pit of Saron, which will unlock access into the Halls of Reflection



[2] Tempering The Blade - enter the Forge of Souls, defeat the Devourer of Souls and use the Reforged Quel'Delar at the Crucible of Souls, right behind the boss[3] The Halls Of Reflection - enter the Halls of Reflection, on normal mode, and defeat Quel'Delar, in order to subdue it

Note: if you can't complete the "Purification of Quel'delar" quest, take both quests, transform the raid into party and complete it; turn in the quest for the weapon of choice, do the final quest to obtain the weapon and in the end abandon the other "Purification of Quel'delar" quest.
